Cranium의 구성
cranium은 14개의 facial bone과 8개의 vault bone 및 6개의 ossicular(小骨) bone으로 구성 되어 있다. 발생 초기에는 어떤 종류의 cranial bone들은 여러 부분으로 나누어져 있었다. 例를 들면 occiput는 4개의 section으로 sphenoid와 temporal bone은 3부분으로 maxilla, mandible은 2부분으로 나누어져 있었으나 성장하면서 suture라 부르는 junction에서 합해지게 되었다. 과거에는 해부학자들은 시체의 dried skull만 연구한 결과 cranial suture를 immovable joint로 보아 왔다. 1978년 Dr. John Upledger는 cranial surgery시 living adult에서 재취한 suture material의 histologic section을 연구한 결과 이들 suture들은 그 안에 blood vessel, sutural ligament, osteogenic layer, fibrous connective tissue, periosteum과 sharpey's fiberfmf 포함하고 있는 살아있는 구조물이는 것을 보고하였다. sutural tissue에 압박을 가하면 sensory input를 자극하여 nervous system에 영향을 준다는 것도 알아냈다. 출생시에 cranium의 길이는 약 60∼65%정도 완성되어 있으나 급속하게 성장하여 5상이 될 때는 거의 full size의 90%까지 성장한다. 넓이는 1∼2살에 크게 영향을 받으면서 각 부분의 형태와 크기에 있어서 연속적인 remodeling change를 하게 된다.

Craniosacral respiratory motion

1939년 Dr. William sutherland에 의해서 발견되어 Dr. Major Dejarnette이 reconfirm한 이래 1975년 Michael and Retzlaff, viola Fryman등의 연구를 비롯하여 cranial bone이 motion을 갖고 있다는 것을 증명하는 문헌은 현재 약 300편 이상 보고되었다.

cranial or primary craniosacral respiratory mechanism은 5단계로 나누어진다.
1) brain, spinal cord 그리고 거기에 부착된 조직 고유의 mobility
2) cerebrospinal fluid의 fluctuation
3) intra-cranial and intra-spinal membrane의 mobility
4) cranial bone의 articular mobility
5) sacrum과 iliac bone사이의 불수의적인 mobility

상기의 craniosacral physiologic system은 다음의 6가지 system에 영향을 주기도 하고 받기도 한다.

5. The endocrine system
6. The respirtory system

cranial bone의 motion은 아주 rythmical한 micro-motion으로 physiologic range는 1분에 6∼12cycles로 나타나며 마치 우산을 천천히 열었다 닫았다 하는 것 같은 감각을 느끼며 이 러한 cranial respiratory rhythm은 호흡이나 심장혈관계에서 나타나는 생리적 motion과는 뚜렷이 구분된다. 이러한 cranial respiratory motion은 cranial respiratory index(CRI)로 표시되며 머리를 가볍게 촉지하면 바로 감지할 수 있다. 숙련된 사람은 이 motion을 인체의 어느 부위에서나 촉지할 수 있다.
CRI는 호흡이나 심장 혈관계처럼 운동이나 심리적 상태에 따라 변화되지 않은 아주 stable 한 motion이다. rate amplitude, and symmetry등을 주의깊게 check하면 진단에 아주 소중 한 정보를 얻을 수 있다. 예를 들어서 CRI가 아주 느린 경우는 brain damage나 anoxia등 으로 인한 coma때에 볼 수 있는 low vitablity, low resistance, high disease succeptibility and degenerative state등을 나타내며 rapid CIR는 hyperkinetic children, high fever or hypermetabolic state혹은 약물에 의한 coma등에서 느낄 수 있다. CMD(craniomandibular somatic dysfunction)증상도 CRI에 영향을 미치는 것과 같이 cerebrospinal fluid에 영향을 미칠 수 있다. 경추의 위치 이상은 vertebral artery에 영향을 미치고 dysfunctional mandibular distortion은 internal carotid artery에 영향을 미친다. cranial motion의 origin은 brain cell로부터 오는 것 같으며 수축과 이완, 팽창과 수축 인체 의 다른 기관이나 폐에서처럼 rythmic base에서 일어난다. 모든 살아있는 세포에는 정상적 인 pulsile rhythm이 있다는 것은 이미 밝혀진지 오래이며 brain에 의해서 생성된 energy는 주로 base bone인 occiput와 sphenoid에 영향을 미치게 되고 occiput는 temporal bone과 parietal bone motion을 control하며 sphenoid는 frontal과 facial bone motion을 control하게 된다.
